What is the easiest way to stop my puppy to from chewing on fingers, and everything else in site.?

I've got this puppy and no matter what I do to divert her attention from chewing/nipping she still chews on everything else. I've tried the firm “NO” and look her in the eyes untill she looks away, and the loud “OUCH” and stick a toy or bone in her mouth, but she still tries to chew on everything else, and now she is starting to nip at my 3 and 4 year old children (she is now pinned up when they are at home). I really need to break her of this habbit (nipping at my children) as soon as possible or else I'm going to need to get rid of her.

Also, any tips on paper training/house breaking would also be greatly appreciated.
